- Arrive 20 minutes before to fill out paperwork. Or print the registration form out and bring it with you to the appointment.
- Special Note: If you are in a crisis situation, please go to the nearest hospital emergency room.
- Allow approximately 1.5 hours for the evaluation.
- Cost is $375 without insurance – Call ahead to check on insurance coverage.
- The cost includes both the assessment and the follow up clinical interview.
- After the evaluation is completed, a follow up clinical interview will be scheduled to discuss the results with the clinician. A treatment plan and/or coordinated referrals are provided to appropriate professionals.
